I hereby agree, of my own free will and without any present compensation or promise of future compensation, to permit Cleveland Rape Crisis Center (CRCC), to produce, publish and release text transcripts, audio recordings, video recordings, edited narratives, and/or photographic representations of me to the general public for the purpose of furthering the mission of CRCC.
I permit video and/or electronic images, and/or photographs of me to be published in agency materials and/or other newsletters promoting CRCC; posted on clevelandrapecrisis.org; distributed to media; or to appear in any other CRCC publication/publicity/presentation to further the purpose described above. I agree to republication by any kind of media. I hereby waive any right to inspect or approve the finished videotape, sound track, or advertising copy or printed matter that may be used in conjunction therewith or to the eventual use that it might be applied.
I hereby release and hold harmless all persons, organizations, and entities, including but not limited to all employees and all elected and appointed officers of CRCC and any other people or organizations that distribute or republish any of these materials at any time and in any form whatsoever, from any liability for any and all subsequent reproductions and/or release of my video, audio, photograph, story, name and/ or quotations, or any excerpts or adaptations thereof. This release has no expiration date.
I understand there is no guarantee that the finished product(s) will include my story, image or voice.
